Direct hair implantation, or DHI, is a popular hair transplant technique for those looking to restore lost hair. Hair loss can be a sensitive subject for many of us, with around 85% of men experiencing it by the time they turn 50. A lot of the time, attempts to stop or slow hair loss down can feel pointless. Thankfully, hair transplantation has come a long way in recent years, with new, minimally-invasive techniques that have left the days of unnatural looking grafts long behind. Despite this, choosing to undergo a DHI hair transplant is still a significant decision.

What Is a DHI Hair Transplant?

Direct hair implantation is an innovative hair transplant technique that involves the transplantation and implantation of hair follicles. The extracted and implanted hair follicles grow in groups of 1-4 hairs and these are commonly referred to as grafts. Your technician will take a graft from a donor site, usually the back or side of your head, and implant it into a recipient area, the bald or thinning part. This differs from other hair transplant procedures in the way that the grafts are implemented. During DHI surgery, the specialist tools allow the implantation stage to happen right after extraction, without the need to first create the canals for the grafts. In other transplant procedures, such as FUE, the canals need to be opened prior to implantation, to allow for the extracted grafts to be implanted manually. Similarly to these other procedures though, DHI is minimally-invasive and produces a natural look with little scarring.

Simply put, this technique involves taking hair follicles from one part of your head and implanting them into another part. This sounds pretty complicated, but it’s actually simpler than you might think. So, how does it work exactly?

Step 1: Initial Consultation

Before getting started with the treatment, we’d first like to get to know you! It’s important for us and for your chosen doctor to see your hair loss, in order to gauge your eligibility for the procedure. To do this, we ask that you send us some pictures of your head from the top, front, left, right, and back. Once you’ve done this, one of our Patient Managers will be in touch to discuss the next steps. We’ll also take time to go through some routine general health questions around:

  • Any existing medical conditions you may have.
  • Any regular medication you take.
  • Your hair loss timeline.
  • Your family’s history of hair loss.

These initial consultations will help your doctor to determine the root cause of your hair loss, to ensure that the DHI hair transplant is carried out in the best possible way.

Step 2: Medical Tests and Checks

Following on from your initial consultation your doctor will carry out a few routine blood tests to ensure that you’re in good health to start the procedure.

Step 3: Anaesthesia and Extraction

Before your surgery begins, you’ll receive local anaesthesia which will numb the area of your head that your doctor will work on. Upon special request, general anaesthesia can be administered instead if you’d prefer to be completely asleep for the surgery, however, this is something that should be discussed with both a Patient Manager and your doctor prior to the treatment. Once the anaesthesia has been administered, your doctor will begin by extracting the hair follicles manually using a special tool that has a diameter of 1mm or less, depending on the patient.

Step 4: Implantation

Once the follicles have been extracted they’re then loaded into an implanter tool, known as a Choi Implanter Pen. This specialist tool allows the follicles to be directly implanted into the recipient area of your head. It also gives your doctor more control over the angle, direction, and depth of the hair transplantation, allowing for a more natural look overall.

Step 5: Post-Op Consultation

After your surgery, you’ll have a post-op consultation with your doctor, in which they can explain any aftercare procedures, clear any doubts, and answer any of your questions. It’s also common to receive a post-op package containing all the necessary shampoos or medication that may be needed following the treatment.Precision instruments called implanters are used for the placement of the follicular units in the recipient area.
There are several available implanters, according to the quantity of hairs in each follicular unit (1 to 4 hairs).
For the 1 hair follicular unit, more precise implanters are used and for units with more hairs higher diameter implanters are used. All of them being, however, always under 1mm in diameter.

What Should I Expect From My DHI Hair Transplant?

Following your DHI hair transplant, your head might be a little red, sore, and tender. This should ease off fairly quickly though and is usually gone by the end of the second week after the transplant.

2 Months Post-Treatment

Between weeks 2–8 it’s likely that you’ll experience what’s known as the ‘shedding phase’, wherein most of the transplanted hair begins to fall out. If this happens, don’t panic! It’s completely normal — occurring in around 95% of hair transplant cases — and is simply a transient reaction to the treatment. Your transplanted hair will recover and begin to grow back in the following weeks and months.

2–6 Months Post-Treatment

Hair growth may be fairly slow during this time. By the sixth month, most patients notice that around 20%–25% of their transplanted hair has begun to grow.

6–12 Months Post-Treatment

Typical growth between months six and nine is around 75%. Alongside this, the transplanted hair usually begins to gain in thickness and density. Nine months after your DHI hair transplant should be the time when the remaining 25% of your transplanted hair begins to grow. However, bear in mind that it may take up to 18 months for the full and final results to appear.
